Imm(Rb, Ri, S) -> Address = Imm + Rb + Ri*S

Imm -> displacement, can be any constant # of bytes
Rb -> base register, any of the 16 int registers
Ri -> index register, any register except rsp
S -> can only be 1,2,4,8

This form is usually seen when referencing elements of arrays.
